Work Schedule: the number of hours worked each week will vary depending on the cases assigned to the Clinical Assessor. Clinical Assessors will work up to 40 hours per week.
The Schaefer Center is recruiting for up to ten contractual Clinical Assessors who will use the InterRAI Home Care assessment tool to conduct in-person initial or redetermination assessments of Maryland residents participating in or applying to certain Maryland programs. The contractual positions are grant-funded and contingent on the continued availability of funding. Current funding ends on June 30, 2016 unless terminated earlier. The Center will consider both full time and part time candidates.
The Clinical Assessors will conduct in-home assessments of assigned clients using the InterRAI Home Care assessment tool, under limited supervision, within specified geographic areas within the state of Maryland. The Clinical Assessors will enter assessment information into a web-enabled form using a laptop or tablet computer. Assessors will schedule, follow-up, and complete the assessment and Plan of Care for each assigned client. Additionally, Clinical Assessors will be available by phone and email to answer assessment questions from involved agencies and local jurisdictions. Clinical Assessors will be required to drive to the assessments throughout Maryland. Limited overnight travel may be required. Clinical Assessors will be required to complete and submit travel expense reports of their travel on a timely basis.
Required Minimum Qualifications
Education: Bachelor’s degree in Nursing or Social Work from an accredited college or university required. Candidates may substitute a Bachelor’s degree in a related field from an accredited college or university and one year of experience as a registered nurse (RN), licensed practical nurse (LPN), or licensed social worker (LCSW-C, LGSW, LCSW) for the required education. Candidates also may substitute two years of experience as a registered nurse (RN), licensed practical nurse (LPN), or licensed social worker (LCSW-C, LGSW, LCSW) for the required education.
Experience: Two years of experience using an assessment tool to collect information and providing accurate documentation and record keeping.
Licenses, Registrations, and Certifications: Candidates must possess a current license as a Registered Nurse (RN), Licensed Practical Nurse (LPN), or social worker (LCSW-C, LGSW, LCSW). Candidates selected for the position will be required to operate a motor vehicle and drive throughout Maryland thus requiring a motor vehicle operator’s license valid in the state of Maryland and daily access to a reliable means of transportation that allows them to drive throughout the state of Maryland. Candidates for the position will be required to pass a background check.
Preferred Qualifications
Preference will be given to candidates who possess a B.S. RN degree.
We also prefer candidates with experience using the InterRAI HC home care assessment tool in Maryland or another state for assessing clients who are elderly, disabled – especially those participating in a Medicaid waiver program. Preference will be given to candidates with experience working with elderly or disabled populations or with intake assessment experience in institutional settings with the elderly, psychiatric, and/or disabled populations.
