Documentation Guidelines

University of Baltimore Campus Services

In order to provide reasonable accommodations for students with disabilities, proper documentation of the disability must be presented to the director of disability support services (for Liberal Arts and Business students) or to the assistant dean of student affairs (for Law School students: Leslie Metzger - lmetzger@ubalt.edu). This documentation is not part of the student's college record. Documentation will remain in the DSS Office for seven years; then it will be destroyed, unless the student requests it be returned.

Guidelines for Specific Disabilities

Guidelines are provided to help students obtain appropriate documentation from qualified professionals. By documenting a student's disability, the DSS office is informed about the impact of the disability, the student's needs, and potential accommodations needed.

The University may request documentation to verify the existence of a disability, to assist in the determination of eligibility for auxiliary aids and services, and to minimize the impact of the disability.

Responsibilities of Students with Disabilities

  • Advance request for accommodations/academic adjustments
  • Advance notice for facilities access
  • Advance submission of proper medical documentation
  • Meet with a DSS staff member for an intake appointment and discussion of potential accommodation(s)
  • Be a self-advocate

The University of Baltimore and the Disability Support Services Office have the following rights and responsibilities:

  • Determine the appropriateness of submitted documentation and requests for accommodation(s) on a case by case basis, based on the judgement of the DSS director
  • Request additional information to determine eligibility for services
  • Keep all information regarding the student's disability confidential
