All contracts must be hand-delivered to Human Resources two weeks before the start date. Please date stamp all contracts upon delivery.
UPDATE: All I-9 Employment Eligibility Verifications will be processed by the Office of Human Resources.
NOTE: If you are renewing a contract and the employee is updating their address, you must submit a new W4 form or use the Payroll Online Service Center.

Adjunct
For a new adjunct, submit to the Office of Human Resources:
- Adjunct Faculty Contract- complete in DocuSign
- W4 form
- MW507 form
- Employee Information Form
- Direct deposit form
- Transcript
- Substance Abuse Policy Sign-Off
- Secondary Employment Form (This form is required if the employee has another full-time State or USM/UBalt job.)
- Other Employment Questionnaire (This form is required if the employee has another State or USM job.)
- Exception Form for Exceeding Overload Limitations (This form is required if regular employee is teaching more than one course or non-UB employee is teaching more than two courses.)
- UBalt Employment Application - complete online
Provide the new adjunct with the following:

- ACA Fact Sheet for Contractual Employees
For a renewal adjunct, submit:

For a renewal contractual I and II, submit:
- Contractual I or Contractual II contract- complete in DocuSign
- Other Employment Questionnaire (This form is required if the employee has another State or USM job.)
- Contractual II forward the renewal letter at least 2 weeks prior to the date of the renewal.
Note: Contingent II employees are eligible for a Cost of Living Adjustment when granted by the State Legislature. Generally, these increases begin on July 1st. Renewal letters must be forwarded to HR at least 2 weeks prior to July 1st, in order to allow sufficient time for HR to set up the new pay rate and for Central Payroll to process the increase.
Graduate Assistant
For a new graduate assistant, submit to the Office of Human Resources:
- Graduate Assistant Contract - complete in DocuSign
- Graduate Assistant Tuition Waiver Form (Send directly to the Bursar's Office, not HR)
- W4 form
- MW507 form
- Direct deposit form
Provide the new graduate assistant with the following:

For a renewal graduate assistant, submit:
