Accommodation Requests
These forms are for eligible students who are already registered with DAS.

Request for Accommodation Letter to be Sent to Instructor/s
- Students must submit a Request for Accommodation Letter to be Sent to Instructors form every semester to receive accommodations.

Alternate Format Textbooks and Course Materials Form
- Students can complete this form to request alternative formats for their readings and textbooks. Examples of alternative formats include EPUB, screen-reader-accessible PDF, and BRF.
- Students can upload inaccessible scanned documents or input information about their readings.

Notetaking Support Services Form
- Students may select their notetaking accommodation preferences for each class in which they are requesting notetaking accommodations.
- Notetaking accommodations are not automatic. Students must complete a new form each semester.
- Eligible students who do not need notetaking accommodations for any of their courses do not need to complete a form.

Accommodated Testing Student Form - Undergraduate and Graduate Only
- Eligible students who wish to have their exam proctored by DAS should complete this form at least 3-4 buisness days before their exam.
- The following students should
not
complete this form:
- Students enrolled in the University of Baltimore School of Law
- Students intending to take an exam that would not be otherwise proctored. For example, an online exam that is not being observed via live video conference.
- The following students should
not
complete this form:
- Eligible students who wish to have their exam proctored by DAS should complete this form at least 3-4 buisness days before their exam.
